Teledermatology is the delivery of dermatologic health care to remote locations by means of telecommunications and information technology. It facilitates delivery of services by a dermatologist at a different time and place than the patient who receives them. Both real-time videoconferences and store-and-forward systems have proven to be highly reliable and accurate in teledermatologic diagnosis compared to traditional face-to-face diagnosis.

A reddish brown eruption


This 3-month-old infant was born with a generalized mottled eruption
comprised of reddish brown maculopapules. The mother of the infant
reported that the eruption would turn bight red and edematous as the
child cried. There was no family history of atopic diatheses. What
would be the differential diagnosis?

It certainly looks like cutaneous mastocytosis Shahbaz.The ideal test is to rub a lesion and see if it urticates.Mother should avoid vigorous drying and should only pat dry.Also avoid aspirin and codeine mixtures which may potentiate histamine release.

I agree with Dr. McColl. If this is generalized, there may be a need to check the wbc periodically. You might want to review the chapter on Mastocytosis on http://www.emedicine.com since there are various forms and some have internal manifestations and some a small risk of leukemia. This case is more extensive than most and I would consult a review.
